The Purpose of the Annual Faculty Review:
- Help each faculty member set individual career goals that are realistic and concrete, and consistent with the missions of the department, school and institution.
- Provide a basis for reviewing/assessing past performance, career development and faculty goals vis a vis the expectations of the department chair and the institution.
- Supplement and, in some cases, quantify information provided in the faculty member's curriculum vitae for administrative decisions which impact salary, promotion, and other faculty benefits.
*Annual Faculty Reviews will not be required for new faculty who were appointed
March 1, 2007 - August 31, 2007.
Deadline: April 4, 2008
Instructions for the Faculty Member:
- Complete sections of this form relevant to your accomplishments and responsibilities.
- Submit the completed form and an updated curriculum vitae to your department chair.
- Request each course/program director to provide a typed summary of your student/trainee evaluations to your department chair (see page 5).
- Your department chair (or a designated division head, section chief) will solicit peer input and discuss with you all of the information, provide written comments and sign the form.
- After reviewing with the chair his/her typed comments regarding your progress and career goals, you should sign the form to indicate that a review has taken place. You may attach a written response if you wish.
- Retain a copy of the completed review record (you will need it for the Six-Year Faculty Review).
Instructions for the Departmental Peer Committee:
- Review the materials provided (including the summaries of the student/trainee evaluations).
- Identify level of performance (category 1, 2, or 3) and comment as appropriate.
- Forward the materials and comments to the department chair
Instructions for the Department Chair:
- Assure objective evaluation by Department Peer Committee.
- Set up a meeting to review and discuss information provided on this form (including the summaries of the student/trainee evaluations, peer evaluations and curriculum vitae) with the faculty member.
- Provide guidance concerning his/her career goals.
- Complete the evaluation by providing written comments about the faculty member's progress and next year's assignments/expectations.
- Submit the completed review packet, signed by you and the faculty member, and the faculty member's written response (if any) to the Office of Faculty Affairs, G.300 MSB, by April 4, 2008 . Please retain a copy in the department.
- The Annual Faculty Review will be used by the School Faculty Review Committee as part of the documentation for the Six-Year Faculty Review.
